Three things, and all three have to be present: a toe box wide enough for toes to spread, a flat platform with no drop from heel to forefoot, and a sole thin and flexible enough for the foot to bend and feel the ground. Drop one of the three and you have a comfortable shoe, not a barefoot one.
You can wear them immediately; you should not run a long route in them immediately. Calves and the small muscles under the arch have been doing less work than they were built for and need a few weeks of short walks and short runs to catch up. Calf soreness after the first outing is expected. Sharp pain is not.
Cool water, mild soap, a soft brush, then air dry away from direct heat. Radiators and tumble dryers shrink uppers and unstick soles. Knit models tolerate a cold gentle machine cycle in a laundry bag; leather and suede do not.
Thin soles are the trade-off that gives you ground feel, and they wear faster than a thick slab of foam. Watch the outsole tread rather than the upper: when the lugs or the tread pattern have gone smooth underfoot, the shoe has done its job.
Measure first, then decide — that is the whole answer, and Sizing & Fit walks through it. Rule of thumb: between two sizes in the barefoot and trail lines, take the larger; in the athletic sneakers, take the smaller.
The toe box is the widest part of the shoe by design, which is unusual and is precisely the point. Feet that have been squeezed into tapered shoes for years usually find the room welcome from the first wear.
